MAKEUP MADHOUSE SERVICE AGREEMENT

Thank you for your interest in the services of Makeup Madhouse! We are happy to have the opportunity to work with you and give you the experience you deserve. Please carefully read the entire agreement and complete the form entirely. We require a non-refundable deposit of 50% of the total expected cost of services at the time of submission to secure your booking. The remaining portion of the balance is due one week prior or the day of your service. Payment methods include Venmo, Paypal, Cashapp, Zelle, Cash, and Check. Terms of Service

Customer Care: We take pride in listening to YOU, our client! Whether you are getting a natural makeover or a full body paint, we want to meet and exceed your expectations. The best place to get in touch is carrie@makeupmadhouse.com if you have any questions or concerns.

 

Makeup Madhouse LLC does not and shall not discriminate on the basis of race, color, religion, gender, gender expression, age, national origin, disability, marital status, sexual orientation, or military status, in any of its activities or operations. In fact, we embrace and encourage all people! This is a safe and positive place. 

 

Privacy & Safety: A verbal or written contract will be given at the time of service for photography release. If you choose not to have any images shared of your makeup or body art, please share that with us and we will respect this.

 

Bookings: To secure a date, a signed contract is required with a 50% deposit due at the time of signing. The deposit is non-refundable under any circumstances. The full balance is due 1 week prior to or on the date of service. Please be advised that dates and times will only be reserved when a signed contract and deposit are received.

 

Parking Fees: Where parking, valet, or toll fees may be incurred, the amount will be included with the final bill and due for payment on the day of the event.

 

Liability: All brushes and makeup products are sanitary and sanitized between every makeup application. The client must report any skin condition and or allergies of anyone in the party to Makeup Madhouse prior to application and, if need be, a sample test of makeup may be performed on the skin to test reaction. Clients agree to release Makeup Madhouse from liability for any skin complications due to allergic reactions.

 

Cancellation Policy: Cancellations must be made 14 days prior to your reserved date or you will be responsible for the full amount of services agreed upon in this contract.

COVID Policy: All deposits are non-refundable, however, if you have to move your service date due to COVID, the deposit amount will carry over to the new date as long as Makeup Madhouse is available for the new date.

 

Makeup Madhouse LLC reserves the right to cancel due to circumstances such as; a result of sickness, accidents, acts of God, and other reasons beyond Makeup Madhouse’s control. In the unlikely instance that Makeup Madhouse is unable to perform its services, the client is entitled to a full refund of any money that has been paid.
