Photo & Media Release

I hereby grant permission to Makeup Madhouse LLC to use my image, likeness, and/or voice in photographs, videos, or other media. I understand these media may be used for promotional, educational, or commercial purposes, including but not limited to social media, websites, print or advertising.

Image Review Period: I understand that I will have 3 calendar days from the date I receive my finalized media to notify Makeup Madhouse LLC in writing of any specific images I do not wish to be used publicly. Written notices should be sent via email to carrie@makeupmadhouse.com. If no written objections are received within this 3-day window, all media will be deemed approved for use.

Subject to the review period above, I waive any further right to inspect or approve the final product, and I release Makeup Madhouse LLC from any claims or liability related to the authorized use of these images.

Skin Sensitivities & Allergies

The paint and products used by Makeup Madhouse LLC are cosmetic-grade. However, individual skin reactions can occur.

Paint may transfer to clothing or personal items during or after the session. While most products wash out, Makeup Madhouse LLC is not responsible for permanent staining or damage to personal property.

I acknowledge that body paint, cosmetics, and FX materials will be applied to my skin. I have disclosed all known skin conditions, allergies, and sensitivities above. I assume full responsibility for any skin reactions, allergic responses, or property damage that may occur, and I agree to participate at my own risk.
